Various treatment modalities for all types of skin problems are available - pigmentation, melasma, acne, rough skin, superficial wrinkles and dark circles. Iontophoresis with galvanic electrodes, which is one of the newest modalities of treating these disorders, is an integral part of these face treatments, which are individually tailored to suit each patient’s needs.

The skin department at Dr. Balabhai Nanavati Hospital boasts of the best medical expertise in the field, as well as the latest surgical treatments available, performed by renowned dermatosurgeons of the country.

Imaging (Isotopic Scans) is carried out with state-of-the-art Siemens Diacam Gamma Camera System, interfaced with an advanced computer capable of creating tomographic slices and other quantitative information.

Routine scans are carried out like Thyroid, Bone, Liver, Renal, Gall Bladder and advanced studies like Myocardial Perfusion scan using thallium and other compounds.

The RIA equipment includes the semi-automatic RIA counter that is interfaced with a computer for the final readings. The department carries out estimation of thyroid hormones, gynaec hormones and tumour markers.




Radiation Oncology
DEPARTMENT OF RADIATION ONCOLOGY

Nanavati Hospital's Department of Radiation Oncology was set up in 1980 to provide radiation therapy to patients. The department is equipped with a mega voltage therapy machine, simulator, computerized treatment planning system and brachytherapy unit. Patients are simulated on conventional simulators as well as on CT and MRI equipment. The images obtained from these sophisticated machines are fed into a recently acquired computerized planning system 'ASHA', a system approved by the FDA (USA).

The department is also very active in the field of research. Multicentric trials with reputed institutes like the International Atomic Energy Agency and Department of Atomic Energy of India have been conducted. We work closely with the Indian Institute of Technology and Bhabha Atomic Research Centre, in our research pursuits.
The Smt. Sarojben Jhaveri Centre for Hyperthermic Oncology and Medicine
Established to impart Hyperthermic treatment, this facility is unique to our centre, and initial studies have demonstrated excellent results, comparable to data published in the west.
Hyperthermia refers to heating of cancerous tissue to temperatures of 410 - 450° C, in order to render cancer cells inactive. Heat alone, or together with radiation and chemotherapy, helps mobilise cell kill rates. Head and neck cancer, and some pelvic tumours, can be effectively treated. The division will soon begin a prospective trial to prove its initial findings, with the support of the Indian Council of Medical Research. A prospective trial to assess the benefits of hyperthermia treatment in HIV patients is also on the anvil.

Welcome to King Edward Memorial Hospital
Founded in 1926, the Seth Gordhandas Sunderdas Medical College (GSMC) and the King Edward Memorial (KEM) Hospital are amongst the foremost teaching and medical care providing institutions in India. The medical college (school) provides training to about 2000 students in undergraduate, postgraduate and superspeciality medical courses; in undergraduate and postgraduate physical and occupational therapy; Masters and PhD courses in various allied specialties. A nursing school is also maintained by these institutions.

With about 390 staff physicians and 550 resident doctors, the 1800 bedded hospital treats about 1.8 million out-patients and 68,000 in-patients annually and provides both basic care and advanced treatment facilities in all fields of medicine and surgery.
Funded mainly by the Municipal Corporation of Greater Mumbai , these institutions render yeomen service - virtually free of cost - mostly to the underprivileged sections of the society
Modern medicine was introduced to India by the Europeans. With the foundation of the Grant Medical College in Bombay in 1845 the 'native citizens' of the presidency were given an opportunity to study western medicine from some outstanding teachers. The college soon gained a world-wide reputation and its founder-principal, Dr Charles Morehead, was invited to help the University of London in formulating its programme for medical education.
Towards the end of the century the old order represented by Sir Robert Grant, Dr Charles Morehead and Sir Jamsetjee Jejeebhoy was replaced by officers of the Indian Medical Service (IMS) who, far from encouraging the best talent of the college, maintained their hold on the prestigious professorial posts. After much hesitation, a few non-clinical professorial posts were opened to non-IMS Indians and Dr Y. G. Nadgir was the first to be appointed to a chair (Anatomy). The fact that an IMS ' officer would be professor ofmateria medica one year and teach obstetrics and gynaecology the next underlined the unwillingness of the authorities to give opportunities to capable Indians.
Nationalist Indians strove to correct this imbalance. Among them was Dr K. N. Bahadurji, the first Indian to obtain the coveted M.D. degree of the University of London. When he was denied the post of Professor of Medicine at the Grant Medical College (an IMS officer holding only the Licentiate of the Royal College of Physicians diploma (LRCP) being appointed instead) he advocated the establishment of a new medical college, staffed by Indians, that would break the monopoly of the IMS. He placed the idea before Sir Pherozshah Mehta - the lion of Bombay - and other members of the Bombay Municipal Corporation. Dr Bahadurji died on 15 August 1898 before his plans could materialize.
GENESIS AND FOUNDATION In 1907, under the Police Charges Act, the work of medical relief within the city of Bombay was entrusted to the Municipal Corporation. In 1909, an ad hoc committee of the corporation decided that 'the time had come for the Seth Gordhandas Sunderdas Medical College in 1925 Photo by Keshav M. Godhia to be associated with the proposed hospital. At the instance of Sir Pherozshah Mehta, the donors also insisted \ that the professors and teachers to be employed should I all be properly qualified independent Indian gentlemen I not in government service. The Municipality approached the Bombay Medical Union for a detailed scheme for the organization of the medical college and hospital. Dr. Jivraj Mehta, just returned from London after obtaining an M.D.degree, was approached by the union. He suggested a radical departure from the traditional design of teaching hospitals in India where isolated blocks housed separate departments. Dr. Mehta proposed that the entire medical college be housed in one large building and the hospital (including the out-patient block) in a separate building. This would facilitate co-ordination between the various departments. The two buildings were to be interconnected by covered corridors so that patients, students and staff could easily go from one building to another during heavy monsoon rains. (The Seth G.S.Medical College and K.E.M. Hospital were the first multistoreyed institutions of their kind. The K.E.M. Hospital was the first Indian hospital housing the out-patient department within the main hospital building).
The plans were submitted to W.A.Pite who had designed the Kings College Hospital in London and was then a leading authority on hospital construction. The local architect was G.Wittet. In those days it was thought that if an architect happened to be an Englishman, he was not only a fit person to draw up plans for a hospital but also to select its equipment. Wittet drew up a long list of equipment to be imported from England, including even ordinary beds for the wards, lockers and mobile screens.
The equipment committee (consisting of Dr Rustom Cooper, Dr P.T.Patel and Col.Hamilton) insisted on obtaining most items from Bombay. Wittet strongly expressed his resentment but was disregarded. When the hospital and the college were formally inaugurated on 22 January 1926, Wittet was presented a gold cigarette cas 5* by the Governor of Bombay, Sir Leslie Orme Wilson, in appreciation of his services. In the very first week, however, a large piece of the plastered ceiling of the operation theatre came down and within the first fortnight, the tiled floor cracked! The total cost of construction of the hospital was Rs.2,527,699 and that of the college Rs 1,364,574.
In making the first appointments to the staff, the Municipal Corporation was largely guided by Dr G. V. Deshmukha very active member of the corporation and also a big noise in the profession (Cooper). Dr Jivraj Mehta was elected Dean of the college and hospital. The first batch of teachers included Dr.M.D.D.Gilder, Dr P.C.Bharucha, Dr A.S.Erulkar, Dr P. T. Patel, Dr G. V. Deshmukh, Dr.R.N.Cooper, Dr V.L.Parmar, Dr. N.A. Purandare, Dr V.R.Khanolkar and Dr B.B.Yodh,who, according to Dr Jivraj Mehta, were individuals of the highest capability and deepest integrity. There was a great bond of striving towards a common aim - ensuring a brilliant success for these institutions. Remember, these were the first medical institutions in the country staffed by Indians at the professorial and other levels and there was a great sense of pride in all of us.

What is Osteoporosis?
Osteoporosis has been called the silent epidemic. It progresses without pain or symptoms of any kind and is often only diagnosed when a fracture occurs. Yet it is a disease that can be prevented and treated and must not be considered a normal or inevitable part of aging.

Osteoporosis literally means porous bone. It is a disease in which the bones become weak, brittle and fragile and are more likely to break. Maximum bone density occurs between the ages of 20 and 30. After 30, as a part of the natural aging process, there is a gradual loss of bone density and strength. Bone begins to break down faster than new bone can be formed.
Of the 6.1 crore Indians who are estimated as having Osteoporosis at any given time, between 60-70% are women. One out of every two women and one out every five men over the age of 65 suffer from Osteoporosis.
W.H.O studies place people of Asian origin at a higher risk of getting Osteoporosis. Indians in fact, are said to present the highest prevalence of Osteoporosis, the Osteoporosis fractures that occur 10-20 years earlier in men and women.
Groups Targeted for Assessment of Osteoporosis
Menopausal women and those who have undergone surgical menopause. The rapid loss of bone density in this group is due to the fact that the ovaries stop producing estrogen, the hormone that protects against bone loss.
Pre-menopausal women with a history or presence of prolonged amenorrhea
Patients treated with exogenous steroids.
Alcoholics and patients with chronic liver disease.
Rheumatoid arthritic patients and those with chronic mobility problems.
Patients with metabolic bone disease such as HPT, Cushing's Disease or CRF.

EQUIPMENT DETAILS - BONE MINERAL DENSITOMETRY (BMD)

Of the 6.1 crore Indians who are estimated as having osteoporosis at any given time, between 60-70% are women.

The K.E.M. Osteoporosis and Obesity Assessment Centre has installed the DPX-MD, the high tech, state of the art Bone Densitometry System from LUNAR. For the very first time in India, whole body densitometry will be possible together with fat assessment.

Considered the gold standard for clinical bone densitometry, the DPX-MD uses dual x-ray absoptiometry (DEXA). Having a scan is a simple process. The patient is merely made to lie back on the comfortable padded scan table and breath normally. The densitometer takes care of the rest. Total body scans takes around 5 minutes; femur, AP and Lateral spine scans take under 1 minute. The DPX-MD provides quite, smooth operation with the patient dose 5X lower than competitive bone densitometer. It is powerful tool in a doctor’s hand. It can help detect osteoporosis before fracture occurs, predict future chances of fractures, determine the rate of bone loss and monitor the effects of treatment over a period of time.

The MRI Centre at K.E.M. Hospital has been operational since 1992 and about 50,000 patients have been scanned to date. The centre, in keeping with it’s tradition to offer the best to patients, now has a new state-of-the-art 1.5T GE Echo speed MRI Scanner. The new unit is equipped with several additional features to make the patient more comfortable; these include an inbuilt music system with head phones for the patient, shorter scan times and ‘flared’ patient friendly design of the magnet. Newer additions, such as the phased array body and spine coils, the endorectal coil, and the peripheral angiography coil, improve the image quality of the scan considerably.
The MRI Centre has installed a special high-end MR machine, and we are the only hospital to offer MRI as a complement to ultrasound for unborn children. The Centre began performing antenatal MRI since 2001, and has performed approximately 80 such scans. All foetal organs can be evaluated and studied in detail. While similar work in this field has been done abroad for some time, this is the first time that such examinations have been performed in India.
What is MRI?
It is a computerized imaging technique, capable of obtaining images of the body in any plane. MRI utilizes strong magnetic fields and radio frequency pulses to generate images, which demonstrates more clearly than other methods, the possible differences between normal and abnormal tissues. The procedure is painless, has no known side effects, circumvents the harmful effects of radiation, and provides the radiologist with high- resolution images that offer additional information when compared to the CT scan. With newer technology, it is also possible to obtain functional and biochemical information of body tissue. Imaging of the heart and arteries is also possible, through a non invasive procedure.

MR Spectroscopy (MRS)
MR spectroscopy is available at the K.E.M.Hospital MRI centre. The study takes approximately 2 to 6 minutes, and can be an add-on to a routine brain MRI. A non-invasive procedure that does not require any injections, MRS provides biochemical information about tissues. Some areas where the procedure provides invaluable information include:
Differentiating radiation necrosis from recurrent brain neoplasm
Differentiating between types of brain tumours, and monitoring their response to treatment
Early diagnosis of Alzheimer’s disease
Pediatric hypoxia, where MRS can define coma status, and often predict the outcome
Inborn and acquired errors of metabolism (brain imaging)
Perfusion and Diffusion
Perfusion and diffusion weighted imaging (DWI) have radically altered the role of imaging in stroke patients. It is possible to diagnose ischemia within a 3-hour window period so that immediate intervention, if required, can be carried out. DWI is a promising technique for the evaluation of patients with multiple sclerosis, and in differentiating some benign intracranial masses.
Functional Imaging
High-speed gradients and EPI techniques on the MR unit allow functional imaging. For instance, it is possible to map out the motor or visual cortices in a particular patient. This technique plays an important role in patients with neoplasm opting for surgery, as well as in psychiatric disorders.
3D Contrast Enhanced MR Angiography
Non-invasive, high-resolution contrast angiography without arterial catheterization is now possible. Due to the high performance gradients and fast 3D gradient Echo pulse sequence, high-resolution 3D (volumes of) image data can be easily acquired in a single breath hold.
All angiographies, other than coronary are also possible on this machine. These include renal, mesenteric, peripheral angiography and portal venography. The contrast used in MRI is non-nephrotoxic, and hence this study can be performed in patients with compromised renal function.


Cardiac Imaging
Fast sequences allow breath hold imaging and greater morphological clarity. Cine sequences aid in evaluating the cardiac motion. Unique feature of arrhythmia detection window makes it possible to image patients with arrhythmias as well.
Musculoskeletal Imaging
It is now possible to study various joints in motion with MR fluoroscopy and cine MR. Kinematics MR can detect:
Entrapment syndrome
Instability
In addition, the machine can achieve a very small field of view, high resolution 1024 matrix and superior fat suppression. This can be used to study
Auricular cartilage
Trabecular micro fractures
Abdomen and Pelvis
Using breath-hold imaging, phased array and intracavitary coils, MRI has revolutionized abdominal imaging. It is particularly useful in
Staging of pelvic malignancies (especially uterus and prostate)
Evaluating pancreatico-biliary tree (MRCP)
Imaging of anorectic fistulae
Uterine masses (fibroids, adenomyosis etc)
In-uteri evaluation of fetal and placental disorders
Interaction
Clinical meetings are held regularly (monthly or bimonthly) in various branches like neurology, orthopedics and pediatrics. This allows better interaction and discussion between the radiologists and referring doctors. The hospital has registered students for DNB (Diplomate and National Board) which is a nationally recognized degree equivalent to M.D.
A number of articles have been published in various national and international medical journals, on the work done in the MRI centre. Also a number of research projects are being carried out.
MRI IN OBSTERICS
Magnetic Resonance Imaging has come in vogue as an imaging modality because of it’s excellent soft tissue resolution, multiplanar capability and lack of ionizing effect. It is widely used for imaging of brain, spine and musculoskeletal system.
Faster techniques have dawned the era of fetal MRI. This paved the way for meaningful imaging of fetus without the need for fetal or maternal sedation. Single-shot fast spin echo (ssFSE) sequences provide evaluation of the fetal organs, especially of the brain in a detailed fashion. Till date, ultrasound has been the investigation of choice for imaging the fetus in order to screen birth defects. But sometimes it reveals only a limited amount of information. This is due to inadequate visualization of specific fetal anomalies as well as fetal position, maternal body habitus and the amount of liquor. In difficult cases, Magnetic resonance imagining (MRI) is an option that adds to the ultrasound diagnosis to delineate the abnormality better. Maternal anatomy can be evaluated as well at the same time without interference by bones, fat or gas filled organs.
MRI research is demonstrating great potential for imaging pregnancy and improving outcomes in a variety of circumstances including cases of -
• Brain or spinal anomalies
• Chest lesions
• Oligohydramnios (less fluid around the baby)
• Secondary abdominal pregnancy
• Masses within the foetal abdomen
• Placental abnormalities
• Multi-foetal pregnancies
SAFETY CONSIDERATIONS
There is no evidence that clinical MR examinations are harmful to the fetus. No mutagenic or adverse effects have been observed after experimental and clinical studies. However, it is prudent to avoid scanning in the first trimester when organogenesis occurs. The approach can be with the help of a team which comprises of the MRI consultant, Ultrasonologist, pediatric surgeon, obstetrician and gynecologist, geneticist and perinatologist.
